1. Start creating a new RGB image. The example on the left is a 288 x 288 pixel example. Click on the Single Row Marquee Tool. Make a one pixel line (which is going to be your main bar code line). Fill the whole selection with the color of your choice.
2. Enter the following settings after you select Filter > Noise > Add Noise:

Amount 400%
Distribution Uniform

Check the on Monochromatic. Hit Ctrl + T. Then drag it upwards, depending on how high you want it to be.

3. If you wish to add text, use the Rectangular Marquee Tool and cut a little bit of the area where you want the text on the barcode. Add the text and enjoy :)
